When I had my knee replaced, I paid the doctor almost $4 k up front, the hospital around $2 k in advance, then the insurance company covered the knee replacement at around $15 k. I did have medicare but didn't list that as my primary insurance. The hospital send medicare a bill, cuz I was over 65 and they paid another $15 k.
